Heart Disease

As the leading cause of death in the U.S., heart disease includes the conditions that can change how your heart functions whether it’s how blood flows in it or how it beats.

Diabetes

There are 3 main types of diabetes, which occurs when your blood sugar, or glucose, is too high. Whether you have type 1, type 2, or gestational diabetes, your body may not be able to produce enough insulin or use it properly.

Lung Disease

Lung diseases are conditions that impact your ability to breathe easily, such as asthma, lung cancer, and chronic obstructive pulmonary disease (COPD). Genetics or air quality can cause these chronic respiratory illnesses.

Chronic Kidney Disease (CKD)

Our kidneys help remove fluid and wastes from your blood. When they have been damaged, they are not able to clean your blood properly. This can lead to chronic kidney disease and even end-stage renal disease.

Hypertension

Also known as high blood pressure (HBP), more than 50% of Black Americans have hypertension. When uncontrolled, HBP can lead to other chronic diseases such as heart disease and chronic kidney disease.
